Demolition Card Application Forms
CCDO/ CSCS Demolition Card Application forms for New Applications/ Upgrades/ Renewals, e.g. if you wish to apply for an Experienced Worker Card.
Please use the CCDO1 form to apply for Demolition Industry Assessment for Labourer or Experienced Worker Level. Also use this form to apply for upgrade Industry Assessments, e.g. Labourer to Experienced Worker.
The CCDO2 form must be used for renewing Permanent CCDO Cards, e.g. Topman/ Supervisor. Also for upgrading Temporary Experienced Worker Cards to Permanent CCDO Cards following NVQ Achievement.
The CCDO3 form must be used when applying for the Demolition Site Operative card.
Please note: all applications must be accompanied with the appropriate touch screen test pass certificate and the Asbestos & Demolition Safety Awareness Certificate valid within the last two years. Asbestos Awareness courses must be carried out by a UKATA or NDTG approved Training Provider.

Touch Screen Test
Please note: The Health & Safety Touch Screen Test is a requirement for all cards within the CCDO Scheme and CPCS Scheme. Therefore, your operative must attend the appropriate test before the assessment takes place.
The NDTG are an approved Touch Screen Test centre offering the full catalogue of Touch Screen Tests. If you wish to book the Touch Screen Test with us, then please complete the Booking Form.

Demolition Card Schemes
Operatives at all levels in the Demolition Industry should hold the appropriate card. The NDTG administers the CCDO (Certificate of Competence for Demolition Operatives) and CPCS (Construction Plant Certification Scheme) card schemes.
